Chronic Disease Management
Use the Chronic Disease Management (CDM) feature to specify care plans, cycle of care for asthma or diabetes, and GP Mental Health Treatment Plans.
Care Plans in the Timeline
When you exit a consult, all care plans, referral letters, and EDC forms are removed from the Consult actions section, and added to a new consult entry in the patient's Timeline.

- Health Summary
  Information entered by a practitioner during a consult is added to a patient's Health Summary.
- Consultations
  Record details of a clinical consultation in the Consult panel of the patient's record.
- Patient Timeline
  A summary of the patient's clinical history is displayed in the Patient Timeline.
- Medications
  Helix uses the MDref medications database provided by AusDI.
- Immunisations
  Before you administer an immunisation to a patient, check the immunisations that have already been administered on the AIR and the immunisations due.
- Allergies
  A patient's allergies can be added to their record.

  - GP Mental Health Treatment Plan
    Use the Chronic Disease Management (CDM) feature to add a GP Mental Health Treatment Plan for patients.
- History & Examinations
  Record details of a patient's clinical history in the History section. Record details of today's examination of a patient in the Examination section.
- Measurements and Assessments
  Record metabolic, pathology and ECG measurements and other assessments in the patient's record as required.
- Recalls
  A Recall is a type of flag recorded against a patient's record that informs practitioners and practice staff of when the patient is due to return to the practice.
- Nursing Requests
  During a consult, add a request for a nurse to complete a nursing action.
- Obstetrics
  Record obstetrics information on the Obstetrics panel.
- Investigations
  Investigations can be requested by a practitioner from within a patient's clinical record.
- Tasks
  The tasks module allows users to send messages (tasks) to other users throughout the medical centre.
- My Health Record
  Securely upload a patient's health data to their My Health Record.

Care Plans in the Timeline

If a care plan has been added for a patient during a consult, the

Recalls for care plan review are added separately to the timeline. To action a recall, click on it in the Patient Timeline icon Patient Timeline. For more information, see Recalls in the Patient Timeline.

Patient Timeline, consult:

To view a care plan, click CDM. For information about navigating the preview, see Document Controls.
If referrals are attached to the care plan, to display a list of assignees, click CDM and select a provider whose referral you want to view from the list. In this case, to view the care plan, select its type in the list, for example GP + TCA.

To review and update a care plan, select Consult icon Consult > CDM icon CDM and for the required care plan, click Review.
